Course Description / Needs Assessment

Disorders of the esophagus, stomach and intestine are the subjects of considerable basic and clinical research over the past several years. New technologies to enhance endoscopic visual observation of surface tissues are allowing more rapid and precise diagnoses for patients with Barrett esophagus and other mucosal diseases. This pre-malignant condition of Barrett esophagus is related to the fastest rising incidence of cancer in white males in this country. There is a lack of understanding of these recently developed technologies and their applications to patient care based on knowledge gained from regular referrals to our clinics for consultation and the limited number of medical articles published. Teaching by experts instrumental in the development, testing and applications of newly developed technologies is clearly needed to improve patient care.

Understanding of the latest research information in the newly developing field of esophagology is essential for optimum patient care, but most recent information is not yet available in either textbooks or literature. Consequently, lecture presentations by faculty experts are most likely to provide useful information that is immediately clinically applicable.

There are no reports yet available in the medical literature on the endoscopic anatomy of the esophagogastric junction and the lower esophageal sphincter that directly relate this anatomy to the technique of botulinum toxin injection. Clinical research on eosinophilic esophagitis, Helicobacter pylori, irritable bowel syndrome and chronic constipation has discovered new aspects of diagnosis and therapy not understood by many physicians simply because of the newness of the reports.

Most gastroenterologists are unfamiliar with the subtleties of pathophysiology, diagnosis and treatment of oropharyngeal dysphagia, which has only recently been considered as an emerging responsibility of their specialty rather than just for speech pathologists and otolaryngologists. There are very few gastroenterology residency training programs in this country that provide adequate training on these disorders. Therefore, training at the postgraduate level is needed to fill this information gap.

This course is designed to update clinicians on the latest diagnostic and therapeutic innovations and interventions for management of esophageal and gastric disorders by providing practical, current information that will be readily applicable to patient care. Newly developed and relatively complex diagnostic and therapeutic procedures, such as endoscopic mucosal resection and endoscopic lumen restoration, will be presented with details of technique that are not available in the medical literature. Extended panel discussions and case presentations have been scheduled to allow time for optimal participant-faculty interchange.
